MEDICAL
INSURANCE :
Campers must provide their medical insurance. Campers will
not be allowed to register without this information.
History:
Before attending camp, each camper must be examined by a
licensed physician and this must have taken place no earlier
than May 1, 2006. An approved school physical is acceptable
and a copy of this completed form must be mailed in with
your final payment in order for your registration to be
complete. |
